YOUR INSURANCE
Your dental insurance is a bonus on top of your salary. It is negotiated by your employer and the dental insurance company. Just like cable/satelite TV, there are specific packages/bundles of dental treatments that are covered. Your employer picks the insurance bundle. These maximums have not changed much in the past 50 years to adjust to the rate of inflation. If your employer gave you "Food Insurance" of $1000 this would offset the cost but not completely cover your yearly grocery bill. Dental insurance perks offset the cost but may not fully cover your yearly cost for optimal oral health.
Any communication from the dental insurance company will go to you directly.
Your responsibility:

1. Understand the details of your plan
2. Provide your dental plan administrator with
a) pre-treatment forms b) claim forms c) any other info
3. Payment to Moose Lake Dental for dental treatment completed

Our role
Depending on the dental problem and the severity, you will be given treatment options.

Treatment recommendations are not based off of insurance coverage or financial status.
​
The best treatment options are based off of scientific data, research, dental school and continued education teachings as well as experience to optimize your oral health.

You will decide the final treatment option.
​
We will provide you the necessary information to enable you to receive the proper benefits under your dental plan.

How it works
We will send an electronic claim immediately called "Direct Billing" to your insurance company. You will have some payment options.
When office administrative costs are minimized those cost savings are directly passed onto you.

The most cost saving option would be to pay with cash or debit
at an overall reduced rate as an "office overhead reduction courtesy"

A. Save Money #1
Pre-pay with cash/debit up to 48 hours prior and save 10%
B. Save Money #2
Pay the day of service with cash/debit and save 5%
C. Credit Card
For example:

Your appointment is completed and the fee is $200.

You pay us $200 and we will electronically submit the claim to your insurance company.

The added bonus for you are your credit card reward points
If your plan pays 100% , your insurance company pays you $200 in a few days that will pay the $200 credit card bill at the end of the month. No money out of your own pocket, and 100% of the reward points.​
D. Four Month Plan

Downpayment the day of service.

3 additional automatic credit card payments.

This option includes a 5% management fee.
